Advertising Request Form

In order to publish an ad in the conference program book, your advertisement must be submitted by September 1 , 2013. The conference program book is an 8.5" x 11" book. All advertisements will appear before the index at the end of the program book.

In order to be printed in our program book, your advertisement must be:

1. Paid for and received by September 1, 2013.
2. Sent to us as e-mail attachment.
3. In grayscale only.
4. The electronic version of your ad must be Windows-compatible. We cannot use, import, or convert Macintosh-formatted files.
5. The electronic file must be sent in PDF format with fonts embedded.
6. Files must be at a minimum resolution of 250dpi.
7. Art services are not available. We do not touch-up or resize advertisements.
8. Ads must not exceed:
   
   
   • Full Page: 7.5 x 10.25 in. portrait
   • Half Page: 7.5 x 5 in. landscape
   • Quarter Page: 3.5 x 5 in. portrait

With the purchase of an advertisement space in the program book, we will provide a complimentary program mailed to your office. Please let us know if you would like this mailed to an address other than the one entered below.
